This is a relief position working varied 8 hour shifts in the Stanford Women's Cancer Center Oncology Department. There is flexiblilty to the hours as needed.The Nurse Coordinator will work closely with members of the cancer care team who provide clinical trials coordination, multispecialty tumor boards, supportive care, palliative care, and survivorship. The Breast/GYN Oncology Nurse Coordinator will assist patients with cancer and their families and caregivers to overcome some of the healthcare system barriers and provide resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer care continuum. Provide patient education, and obtain or develop oncology-related education materials for patients, staff, and community members as appropriate. The Nurse Coordinator will provide symptom Triage Management, act as part of the multidisciplinary team to provide coordination and continuity of care throughout trajectory of care, be the consistent and primary contact for patients and their families, provide guidance as patients transition from one therapy to another, provide side effect management, coordinate clinic appointments and assist with referrals to ensure it is done in a timely matter. The nurse coordinator effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds and promotes awareness of clinical trials to patients, families, and caregivers. The Nurse Coordinator accompanies care team in patient visits, collaborates with care team about treatment plan and translates complex medical terminology and answers questions. Nurse Coordinator describes a variety of specialized nursing positions that provide assistance in planning, coordinating, and delivering patient care for selected patient populations across the continuum of care using the nursing process, ensuring compliance with physician orders and established policies and procedures. Nurse Coordinators provide coordination and care of patients requiring assistance and support in accessing and managing their health care needs. They collaborate with other health care providers to ensure continuity of care and coordination of a wide variety of patient services. They will use performance improvement methodology to design data collection systems / processes and identify opportunities to improve care / service for patients. Since our 2010 launch in Denver Public Schools, Blueprint has expanded across the country and partnered with more than 100 schools serving over 20,000 students in East St. Louis (IL); St. Louis (MO); Oakland (CA); Leadville (CO); and in Boston, Holyoke, and Salem (MA). About the Blueprint Math Fellows Program The Blueprint Math Fellows Program provides intensive, small group math tutoring to students as part of their regular school day. Our tutorial sessions address gaps in foundational mathematics knowledge and promote each student’s ability to problem solve, think critically, and access grade-level coursework. Tutorial groups are small and personalized. Each full-time tutor, known as a “Math Fellow,” works with no more than four students at a time. Tutorial sessions are 45-60 minutes long, highly structured, and research-based. The Math Fellows Program provides each student with an additional opportunity to understand and master math concepts every day of the school year. Our Math Fellows are also AmeriCorps members and, through their year of national service, make additional contributions to their school and local communities as club leaders, volunteer service day organizers, and coaches. The Math Fellows Program has also become an innovative teacher pipeline. Approximately one out of every four Fellows becomes a full-time teacher, often in the schools where they served as Fellows. In August 2016, Blueprint launched a partnership with Oakland Unified School District to implement the Blueprint Math Fellows Program. In the current academic year, the program has 18 full-time Math Fellows serving more than 450 students in seven middle schools.
