Clinicians will be responsible for supervising the day to day operations and ensuring the comprehensive provision of high quality. They, in conjunction with the Case Managers, will provide holistic and comprehensive case management services to runaway and homeless youth including: intake assessment, emergency shelter and critical basic needs resources (food, housing, clothing, hygiene supplies, and risk reduction materials), benefit assessment, educational needs through the McKinney-Vento Act, goal setting, weekly and long-term case plan development, progress monitoring, and aftercare plans. Additionally, the clinicians will be responsible for encouraging resolution of family problems (when appropriate) through home visits, counseling and therapy. Other responsibilities include: Comprehensive Youth Services Model: Operate within a framework of positive youth development, trauma-informed care and evidence-informed practices in order to foster social support and help youth reframe traumatic events while building a sense of resilience. Access to emergency shelter: Coordinate runaway, homeless, and street youth with safe and stable housing and individual, family, and group counseling as appropriate. Provide street-based services; home-based services for families with youth at risk of separation from the family; drug abuse and education and prevention services. Gateway Services: Provide food, drink, referrals to shelter, clothing, transportation, and hygiene to prevent malnutrition and ill-health while building trust with youth they encounter in order to identify youth who are at-risk of becoming victims of sexual exploitation and human trafficking. Assessments:  Implement standardized methods to assess each youth’s situation at contact, such as immediate needs, physical mental, and behavioral health, connection to family, safety, access to resources, issues of neglect, or abuse and other risk and protective factors impacting well-being and self-sufficiency; addressing the unique needs of subpopulations such as youth who are pregnant and parenting; running from a foster care setting; sexually exploited; in need of substance abuse or mental health services; or LGBTQ.  Coordinated Case Management: In conjunction with case managers, ensure that youth have the support in accessing a variety of services that are coordinated across multiple systems to include; accessing a range of options for trauma informed behavioral health services; the development of an individualized service plan tailored to meet the specific needs of the youth; and individual, family, and group counseling; Set goals, provide direct services e.g. clothing, food, shower, etc., and make proper referrals for housing (if eligible) Coordinated Entry System (CoC) which will assess youth’s housing options and make appropriate referrals with the appropriate offices to enroll the eligible youth in health insurance and in Affordable Care Act, Medicaid, CHIP and other benefit programs (e.g., Supplemental Security Income program (SSI)/Social Security Disability Insurance Program (SSDI), Temporary Assistance for Needy Families (TANF), and the Supplemental Nutrition Assistance Program (SNAP).  Case supervision sessions: To be conducted by the clinicians weekly to review current cases and the types of counseling and other services, which are being provided.  Follow-up/Aftercare:  Provide aftercare services to help foster permanent connections with caring adults, which include providing or referring youth and their families to continued family counseling through area Family Services outpatient programs. Following the youth’s return to home or placement in alternative living arrangements, the clinicians will provide follow-up which consists of establishing an exit plan with youth to include permanent placement planning that will help ensure that youth will have the opportunity to make informed decisions about the support and services they need, develop a plan for permanency, and identify and achieve their personal goals.  Continuum service linkages: Coordinate with others such as government, nonprofits, other outreach teams, referral providers, and service providers to ensure their ability to serve the homeless youth population to include the pertinent service linkages.
