The Coordinator of the Inpatient Wound Care Program is responsible for planning, assessing, measuring and implementing wound care and ostomy performance improvement programs within the system. The Coordinator would provide relevant education to medical staff, system staff and the governing body. Facilitates performance improvement activities in wound care, diabetes management and ostomy care. Acts as a resource to the administrative team, department managers and medical staff with respect to the quality of care provided for wound care in the inpatient setting. FLSA: EXEMPT DUTIES AND RESPONSIBILITIES: Demonstrates effectiveness in planning and implementing performance improvement in wound care. Ability to educate patient and families about wound management, both from a clinical and functional standpoint Provides for professional growth and development of patient care staff through identification of needs, sponsoring training programs, encouraging education efforts and evaluation of same Participates in performance improvement, including utilization and promotion of familiarization with performance improvement standards and programs Demonstrates ability to educate and support clinical staff responsible for the care of persons diagnoses with Wounds Develops skin care programs to prevent skin breakdown of patients. Practices good guest relations with patients, visitors, physicians and staff, assists as needed Communicates appropriately and clearly to physicians, staff, management and administrative teams in regard to best practice in wound care and program implementation. Demonstrates awareness of the responsibilities of the position and interfaces well with all members of the healthcare team to insure best practice and positive outcomes in regard to wound management. Accurately demonstrates use of the database systems to document occurrences, medical staff review functions, committee review an action. Compiles reports for committees and administrative teams. Maintains a good working relationship both within the department and with other departments. Ability to perform wound assessment, reassessment and management for all patients and per policy. This includes neonate, pediatric, geriatric and the general population. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate as well as staff. Provides for professional growth and development of patient care staff through identification of needs, sponsoring training programs, encouraging education efforts and evaluation of same Provides leadership and direction in accordance with organizational goals and objectives Attends seminars, conferences, continuing education classes in the areas of diabetes/wound prevention, diabetes/ wound management and ostomies Participates in professional societies. Stays current with changes, new advances/treatment in wound and ostomy care. Is a resource person to physicians, nursing and other hospital staff. Provides in-services on diabetes and wound management. Cross trained to support Inpatient Diabetic Wound Educator/Coordinator as well as the Outpatient Wound Clinic Demonstrates knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care and incontinence. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians and other healthcare providers. Consults other healthcare professionals/departments as appropriate to provide interdisciplinary approach to the patient's needs. Ability to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan for effectiveness. Demonstrates ability to perform treatments and provide services to level licensure. Identifies and addresses psychosocial needs of patients and families; communicates with social service/discharge planner as appropriate. Consults other healthcare professionals/departments as appropriate to provide interdisciplinary approach to the patient's needs. Documentation meets current standards and practices. Assumes responsibility for own personal continuing education and developmental needs Integrates research findings into clinical practice. Knowledgeable of surgical procedures and postoperative course of patients undergoing bowel surgery. Provides education to all patients undergoing surgery that result in an ostomy. Provides pre- and post-op assessments and patient/family education. Knowledgeable of the anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems. Utilizes principles of current wound management theory when providing care to patients with acute and chronic wounds. Education/Experience Requirements: Graduate of accredited school of nursing; Bachelor’s of Science required, Masters preferred Current state Registered Nurse Licensure Five years clinical experience in specialty area Certification in Wound Nursing preferred Current BCLS certification. Experience with data analysis, research and outcomes measurement preferred Language Skills: Ability to effectively communicate in English, both verbally and in writing. Other Skills: Basic computer knowledge, data entry and/or word processing.
