Identify engagment strategies, considering the person's situation and preferences, which can be utilized by the team. Screen, assess and reassess Person for risk and protective factors using agency risk assessment protocols and tools. Oversee the development and implementation of Safety Plans and Crisis Prevention and Response Plans and develop appropriate critical need interventions and transition plans. Build relationships with Persons and develop shared understanding of Persons' goals and the steps necessary to achieve the goals. Ensure timely completion, with Persons served and other members of the Integrated Team, the Comprehensive Assessment and other clinical assessments as needed. Participate in the development and implementation of Treatment Plans and Treatment Plan reviews. Provide direct intervention with the Person served in line with the Person's Treatment Plan. Develop and facilitate clinical and support groups based on the needs and preferences of Persons served. Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with people served and family members. Demonstrate respect for all Persons regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity. Monitor person's needs and progress; design and deliver rehabilitative interventions to assist person in building and using skills. Provide clinical consultation to programs, staff teams, and Persons served on an ongoing basis; respond to serious events as needed. Provide modeling, supervision, and training to direct care staff, especially related to evidence-based interventions. Develop, coordinate, and deliver trainings in conjunction with the Integrated Team members, as needed to address the clinical learning needs of staff (e.g. orientation, crisis intervention, developing a clinical perspective, etc.). Collaborate with applicable CAre Coordination Entities, Emergency Service Providers, outpatient clinics and other providers/entities to share information and ensure that the needs of the person are fully met. Develop productive affiliations and partnerships with other community-based providers to enhance the agency's service delivery network. Provide support and assistance to persons through Care Transitions and other life changes, making such changes as seamless as possible. Assist Program Leaders to identify and evaluate referred Persons' risk behaviors and develop appropriate transition plans and interventions. Coordinate risk management data; assist in the development and implementation of corrective action plans in conjunction with other members of the Integrated Team. Support and monitor the implementation of identified clinical approaches and evidence based practices ascribed to by the agency. Participate in On-Call rotation and critical incident review process. Obtain and maintain MAP certification Attend, participate in, or facilitate meetings, trainings, and supervision as assigned. Perform other duties as assigned. Qualifications Master's Degree in social services field. The Bridge provides excellent supervision (both individual and group), as well as various trainings in which to obtain CEU's, and opportunities for advancement. The Bridge is an eligible employer for the Public Student Loan Forgiveness Program and provides some tuition reimbursement for continuing education. A new 6 bed mobile respite and site based respite program is opening. It will be located in Gardner, Ma. In addition to providing on site mobile respite services, the program will also work with individuals in the community providing short term support services. These services are meant to identify and asses the immediate needs of the individuals referred by DMH, and provide short term support in achieving their more immediate needs Responsibilities Responsible for physical assessments, maintenance of health records and planning and implementing educational groups with individuals served and staff related to each individual's health and disease processes. Assist staff with the implementation of the Medication Administration Program (MAP) and provide training to staff to meet the regulations. Support and actively participate in agency-wide wellness initiatives. Assist staff with appointment scheduling, follow-up with physicians, and hospital discharge planning. Complete required documentation in a professional and timely manner. Eligible candidates must have a solid medical-surgical background due to many of our individuals having co-occurring diagnoses. This nurse will also coordinate care, attend medical appointments with the individuals, review medications, and work directly with the individual's served. Qualifications Eligible candidate must be a registered nurse (RN) and BSN is preferred. Excellent written and verbal communication skills and ability to collaborate with multi-disciplinary team members required.
