The interdisciplinary team works to reduce the burdens associated with life-limiting illness by enabling patients to have the best quality of life possible throughout the course of serious illness.After full onboarding and complete training, this position will also include working with hospice patients and the hospice physician providing expert management of complex symptoms in the home/nursing home setting. Visiting patients, in conjunction with the hospice nurse, at their place of residence to complete symptom assessments and hospice recertification evaluations will be a part of this position.Services Provided/Responsibilities:* The NP/PA will consult with the patient and their loved ones to help delineate their goals of care and aid in complex medical decision making.* Educate patients and family to promote understanding of underlying disease process and expected future course of the illness.* Optimize symptom control (pain, nausea, dyspnea, etc.).* The NP/PA will support Hospice services and collaborate with the Hospice Physician including visits to the patient's residence.* Discuss advance care planning with patients and families.* Assist end-of-life patients and their families in preparing for life closure.* Promote collaborative relationships between hospital team, nursing, and other healthcare professionals to ensure whole-person care.* Co-manage the patient's plan of care and inform primary hospital physician.* Assist the department in clinical practice guidelines/standards in support of quality Palliative Medicine as directed by supervisor and physician lead.* Educate and mentor staff/students/ medical residents.* Promote a system of care that fosters timely access to Palliative Medicine services.QualificationsNurse Practitioner - Master's degree in nursing required, Doctorate of Nursing Practice (DNP) preferred; ORClinical Nurse Specialist - Master's degree in nursing required; ORPhysician Assistant - requires completion of baccalaureate degree and an accredited physician assistant educational program, master's preferred.Certification by a national nurse certification organization acceptable to the Minnesota Board of Nursing.Physician Assistant certification by the National Commission on Certification of Physician Assistants.Additional qualificationsExperience with complex symptom management and working with the seriously ill patient preferred.License or certificationCurrent license in the state of Minnesota or the ability to obtain license to practice.Currently possess or ability to obtain DEA.Basic Life Support (BLS) certification required and any other life safety certification as required to obtain privileges.To be considered for this position please upload 2-3 letters of reference, letter of interest and current CV with your application.Mayo Clinic and Mayo Clinic Health System employees are asked to also upload their last 3 performance evaluations.Exemption statusExemptCompensation DetailEducation, experience and tenure may be considered along with internal equity when job offers are extended.Benefits eligibleYesScheduleFull TimeSchedule detailsMonday - Friday, days.Weekend scheduleAnticipated future holiday/weekend coverage.
