POSITION OVERVIEW The Family Support Worker/Specialist provides parenting education services to families with children ages 0-5. Families are referred by the San Francisco Human Services Agency, Family and Children Services, and community organizations. The In-Home Services program utilizes the SafeCareĀ® parenting education model of evidence-based service delivery. Starting hourly wage is $23.00. Will only consider candidates in San Francisco Bay Area who meet our requirements. RESPONSIBILITIES Instructing parents in structured parenting education practices and providing case management as appropriate Conducting baseline and follow-up assessments Teaching parents child preventive health measures, how to identify symptoms of childhood illnesses and injuries, and appropriate responses including the use of health reference materials, charts, and basic health supplies Assisting parents in identifying and eliminating safety and health hazards Instructing parents in the provision of activities that are motivating, promote positive interactions with their children, and prevent troublesome child behavior Providing supportive counseling to parents including positive and corrective feedback Assisting clients in addressing challenges utilizing a structured problem-solving method Maintaining timely and thorough case records Participating in training and coaching including detailed feedback Participating in team activities including home-visits shadowing/being shadowed APPLICATION PROCESS To apply for this position, please submit your resume with a cover letter. IOA is currently seeking a Clinical Supervisor- Multipurpose Senior Service Program (MSSP) whose primary responsibility is to provide clinical supervision of MSSP Care Management staff including Masterā€™s Level Social Workers, bachelorā€™s Level Social Workers, and Registered Nurses. MSSP provides interdisciplinary care management services to frail, low-income seniors to assist with their ability to continue living independently in the community. ESSENTIAL JOB FUNCTIONS: Responsible for the clinical supervision, training and evaluation of the Care Management staff working in MSSP. Participates in the hiring and termination of MSSP Care Management staff. Facilitates and oversees day-to-day operations; insuring that appropriate systems are in place. Ensures program and staff compliance with all regulatory requirements as set forth by California Department of Aging. Insures high quality, timely care and service delivery. Implements and monitors multiple systems, including records, clinical documentation, and personnel. Ensures that documentation is compliant with regulations, policy and procedures. Organizes and conducts weekly care plan meetings and supervision. Facilitates the orientation and training of all new staff, including joint home visits, supervisory meetings, and documentation review. Responsible for assessing the educational needs of staff, developing and coordinating an education and training plan that assures a high level of skill and competency in the understanding of case management and community resources. Develops and maintains collaborative working relationships with community individuals, groups and organizations; takes advantage of opportunities to educate the public about MSSP services. Responsible for remaining current with changes in San Francisco community resources, Medi-Cal and Medicare regulations and procedures that will impact the practice and direction of the program; acts as a resource to MSSP staff. Ensures that MSSP waived services funds are used as a last resort and that all community resources and referrals are made prior to the authorization of MSSP funds. Provides back-up coverage for clinical staff. Works with local schools to secure and supervise social work interns. In collaboration with the MSSP Program Director, participates in establishing monthly, quarterly, and annual goals and objectives relevant to the needs of the program. In collaboration with the MSSP Program Director, conducts annual review and revision of program policies and procedures. Responsible for expanding personal knowledge, understanding of care management, family systems, geriatrics, addiction and other relevant heath care issues. Actively incorporates the ethical standards of the National Association of Social Workers into all aspects of interactions with others. Understands and applies the regulatory and procedural requirements of MSSP as well as the policies and procedures of the Institute on Aging. Attends continuing education classes and/or inservice training to increase knowledge, skills and attitudes related to case management, gerontology, family and community systems and other areas relevant to the MSSP client population. Provides mentorship, consultation, and training for all new MSSP CM staff and interns. Assumes responsibility for some of the programā€™s most challenging clients. Participates in program leadership meetings and takes on program development projects as assigned. Acts as liaison between MSSP and some other providers serving large numbers of MSSP clients such as primary care clinics, housing organizations etc. All other reasonably related responsibilities as assigned. REQUIRED QUALIFICATIONS: MSW. (masterā€™s in social work) degree or a related field (nursing, psychology, counseling, rehabilitation, gerontology, or sociology) from an accredited school. LCSW preferred. Two yearsā€™ experience working with frail elderly required. Experience with and understanding of the medical and psycho social problems of frail elderly. Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems and teams of health professionals. Demonstrates case management skills and experience in the community health care delivery system. Demonstrates ability to supervise care management staff and provide coaching / mentoring to staff. Demonstrates ability and willingness to take on program development projects. Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks. Computer literacy required. Ability to comprehend complex written materials and verbal instructions. Ability to speak and write English clearly. PHYSICAL & LANGUAGE REQUIREMENTS: Ability to lift, transfer, push/pull, maneuver, and reposition up to 25 lbs. Ability to reach, bend, and walk. Five finger manipulation.
